Minnesota's nonpartisan Legislative Auditor just dropped the most damning fraud report of her career — and it lands squarely on Governor Tim Walz's Department of Human Services.

Auditor Judy Randall's office found that DHS's Behavioral Health Administration handed out 𝐦𝐨𝐫𝐞 𝐭𝐡𝐚𝐧 $𝟒𝟐𝟓 𝐦𝐢𝐥𝐥𝐢𝐨𝐧 in mental health and addiction treatment grants to 830 grantees between mid-2022 and the end of 2024 — with pervasive failures to verify who was actually getting the money or what they did with it.

Then came the part Randall says she's never seen in nearly 30 years on the job: when auditors started asking questions, DHS staff didn't just admit the gaps — they 𝐦𝐚𝐧𝐮𝐟𝐚𝐜𝐭𝐮𝐫𝐞𝐝 𝐩𝐚𝐩𝐞𝐫𝐰𝐨𝐫𝐤 𝐭𝐨 𝐜𝐨𝐯𝐞𝐫 𝐭𝐡𝐞𝐦. Investigators found documents created from scratch and backdated to make it look like the work had been done on time. In one case, a record was backdated by 𝐭𝐰𝐨 𝐟𝐮𝐥𝐥 𝐲𝐞𝐚𝐫𝐬.

“𝘐𝘵 𝘢𝘱𝘱𝘦𝘢𝘳𝘦𝘥 𝘵𝘰 𝘣𝘦 𝘢 𝘴𝘺𝘴𝘵𝘦𝘮𝘪𝘤 𝘦𝘧𝘧𝘰𝘳𝘵” to fabricate documentation that never existed, Randall said — calling it the most egregious thing she's encountered at the Office of the Legislative Auditor.

The agency couldn't even produce proof that required monitoring visits happened. Of 51 grant agreements reviewed, more than half the progress reports were missing or overdue, and DHS has no documentation at all for two dozen supposed site visits covering 11 different grantees.
