Today I attended a patient bleeding from an arterio-venous (AV) fistula. Although it’s not the commonest of calls for ambulance service, it’s the second in 2 months for me. A potentially life threatening bleed and treated as pictured. Tip: A lid from a sample pot (unused) is something which you may have to hand on an ambulance.

We are so good at abandoning and rushing to something “new” in the #NHS

While having visitor #dialysis last wk I filmed my #selfneedling for interest. Note this is buttonholing with blunts, I usually do nocturnal home HD but switched to twilight for holiday. If I was sleeping I’d use the chevron technique. Cleaning & scab removal not filmed 1/2
Many of our systems of accountability are wrong: they punish people when it's the system, not the person, at fault. We need a "just culture" where we understand that error happens & ask people to take accountability to do the right thing. Message/sketch: @rob_dimeo via @tnvora
