We hit a real agent-runtime failure mode today:

A tool-mediated turn stayed in-flight long enough to trigger duplicate/replayed inbound handling. The original run then aborted, and recovery crossed onto the wrong premium model lane instead of staying pinned or failing closed.

We contained the path, tightened routing guardrails, and revalidated with controlled tests.

Main lesson: production agent systems need hard dedupe, lane pinning, precise abort telemetry, and cost-safe recovery semantics.
