Fluid accumulates in the pericardial sac → intrapericardial pressure rises → the cardiac chambers cannot fill properly → stroke volume and cardiac output fall.

Classic presentation includes hypotension, raised JVP and muffled heart sounds (Beck's triad), along with tachycardia and pulsus paradoxus.
ECG may show low-voltage QRS or electrical alternans, while echocardiography is key for rapid diagnosis.

The principle to remember: it's not simply how much fluid is present-it's how quickly the pressure builds.

An attempted jailbreak has been foiled at Ruai Police Station after officers discovered pliers and a pair of shears hidden inside a plate of rice and meat allegedly being delivered to three remanded suspects.

Police arrested 18-year-old Christine Njoki Gitau after she arrived at the station claiming she had brought food for Emmanuel Bosire, Ismael Arisa and Brian Gatheru, who are being held over allegations of robbery with violence and gang rape.

A routine inspection of the food reportedly led to the discovery of the tools, believed to have been intended to help the suspects break out of the cells.

Christine was arrested as police launched investigations to establish how the tools were obtained and whether other people were involved in the alleged jailbreak plot.
