The human intervention bit is indeed vague, my guess is it probably means castration. It is not likely to mean changing dress and presentation, that is banned in Jewish law.

That may be similar to trans issues in certain ways but I don't think it's the same issue and I don't think that concerns gender, as some use the word anyway.
