I am writing to formally file a complaint regarding a serious issue that resulted in my family being denied travel on our scheduled flight from Tampa, Florida to Cali, Colombia on 2nd of may, 2026.

Please note that I have also filed a formal complaint with the U.S. Department of Transportation due to this incident involving denied boarding caused by airline error.

• We are also losing valuable vacation days at work with PTO that cannot be recovered and inability to return to work to stop PTO from being used.

Requested resolution:

I am requesting:
1. Rebooked to today’s May 3,2026 flight or Full refund of all tickets.

- The error originated from Avianca’s reservation handling process.
- We acted in good faith to correctly add our infant.
- No alternative solution, rebooking, or same-day accommodation was offered.
- We incurred significant financial losses due to missed travel.

We believe this constitutes denied boarding due to airline error and failure to provide reasonable assistance or accommodation. As a result, our entire family was unable to travel. We have filed two separate complaints with the following reference numbers: #33120907 and #33122165

At the airport check-in counter, Avianca staff (Omar & Ronnie) informed us that they were unable to process the infant due to this system error. We immediately contacted customer service, but no effective solution was provided.

We purchased tickets for three passengers and subsequently contacted Avianca to add our infant to the reservation. The infant was placed under a separate booking, which, as we later discovered at the airport, was incorrectly associated with a “phantom adult” and not linked to us.
