Dr. Bienfait Mumbere presented a compelling case titled: "T-tube insertion as a salvage procedure for failed primary repair in penetrating esophageal injury complicated by sepsis"—a study from a low-resource setting.
Clinical Presentation & Challenges:
• The Case: A 28-year-old female presented to Lira RRH following a penetrating neck injury (assault) with saliva and food leakage.
• Initial Management: Despite primary repair, the patient developed localized cellulitis and systemic sepsis.
• The Intervention: On POD 3, surgical re-exploration revealed two perforation points. A T-tube was placed as a controlled fistula (salvage), along with a feeding jejunostomy improvised using a urinary catheter.
• Resource Limitations: Due to the prohibitive cost and lack of Total Parenteral Nutrition (TPN), parenteral feeding was managed via the improvised jejunostomy tube.
Outcome & Conclusion:
• Results: A barium swallow at 10 weeks post-op confirmed no leaks or stenosis.
• Success: Following T-tube removal, the patient achieved a full recovery.
• Takeaway: While esophageal injuries carry high mortality risks in low-resource settings, innovative salvage procedures like T-tube drainage can be life-saving when primary repairs fail.
