When weight loss is matched, does diet composition still matter?
In this randomized clinical trial, 55 adults with obesity, prediabetes, and fatty liver were assigned to ketogenic, Mediterranean, or very-low-fat plant-forward diets. Metabolic outcomes were compared after ~10% weight loss.
๐น Liver fat fell 67% with the ketogenic diet vs 45% with both other diets.
๐น Hepatic insulin sensitivity improved 2โ3 times more with the ketogenic diet.
๐น 24-hour insulin levels fell 74%, compared with 44% and 27% on the Mediterranean and very-low-fat diets.
๐น Prediabetes remission occurred in 50% of the ketogenic group vs 29% and 7% in the other groups.
The findings suggest that macronutrient composition may influence the metabolic benefits of weight loss, particularly liver and glucose metabolism.

Can a nutrition-focused, remote care program reduce liver disease risk in high-risk adults?
In a large U.S. matched cohort study (n=10,062), Virta Individualized Nutrition Therapy (VINT) โ a telemedicine program designed to achieve nutritional ketosis and promote metabolic health โ was compared with usual care.
Over an average of 2 years, the VINT participants had a consistently lower incidence of liver-related outcomes:
๐น ~39% lower relative risk of any liver-related diagnosis (HR 0.61)
๐น ~62% lower relative risk of progressive liver disease (MASH and beyond; HR 0.38)
๐น ~67% lower relative risk of advanced liver disease (HR 0.33)
๐น ~75% lower relative risk of liver-related complications (HR 0.25) ๐นGreater weight loss was also associated with lower risk: โฅ15% weight loss, ~34% lower relative risk (HR 0.66)
Despite similar medication use and healthcare utilization, fewer liver-related events were observed in the VINT participants.
These findings support a lifestyle-first, metabolically targeted approach as a scalable strategy to reduce liver disease risk.

This case report describes the experience of a 60-year-old woman with recurrent pancreatic ductal adenocarcinoma (PDAC) who stabilized her disease for 9 months using a ketogenic diet after declining further chemotherapy due to side effects. PDAC has a poor prognosis, and survival after recurrence is typically less than 1 year.
After initial chemotherapy and surgery, she completed additional chemotherapy and radiotherapy for lung metastases. When the cancer progressed again, she began a strict ketogenic diet (<50 g carbohydrates, high fat with medium-chain triglycerides), with meals supervised by a dietitian for 3 months. Urine ketones were monitored daily.
Key outcomes:
๐นDisease remained stable for 9 months on the ketogenic diet alone; lung metastases showed minimal size change.
๐น Tumor marker (CA19-9) levels remained steady.
๐น The diet was well tolerated, with no reports of hunger, hair loss, or other side effects.
๐น Disease progressed at 12 months, at which point chemotherapy was reintroduced.
๐น The patient lived 24 months after starting the diet.
๐น The tumor showed high GLUT-1 expression, suggesting sensitivity to glucose restriction.
This case highlights the potential role of ketogenic therapy in select patients with pancreatic cancer, although more research is needed.

๐ง Can certain dietary approaches slow biological aging in multiple sclerosis (MS)? A new secondary analysis suggests it might.
Researchers conducted a secondary analysis of two trials to evaluate three dietary approaches in people with MS, using metabolomic age (mAge), a biomarker based on blood metabolites, to measure biological aging.
๐นA 6-month ketogenic diet (low-carb, high-fat) reversed biological aging by about one year per month.
๐น An 8-week intermittent calorie restriction plan (two very-low-calorie days per week) reduced mAge by an average of 6.6 years.
๐นIn contrast, daily calorie restriction (with similar weight loss) had no effect on mAge.
These findings suggest that the benefits go beyond calorie deficit alone, potentially involving ketone production, mitochondrial function, inflammation, and gut microbiome changes.
This positions mAge as a useful biomarker in MS research and supports dietary strategies as possible adjuncts to treatment. However, larger and longer trials are needed to determine clinical relevance.

New research suggests that a ketogenic diet may enhance the effectiveness of metabolic therapies against pediatric high-grade glioma.
In this preclinical study, juvenile mouse models of high-grade glioma received a ketogenic diet (KD) alone or combined with the metabolic drugs mebendazole (MBZ) or devimistat.
Key findings:
๐น KD lowered blood glucose, increased ฮฒ-hydroxybutyrate (BHB), and reduced tumor invasion
๐น MBZ + KD reduced tumor growth and invasion while prolonging survival compared with KD alone
๐น Devimistat demonstrated in vivo benefit only when combined with KD
๐น Both MBZ + KD and devimistat + KD significantly improved survival in a second glioma model
๐น KD helped maintain body weight, with no evidence of obvious MBZ-related liver toxicity
These findings are encouraging but preliminary. Because this was a preclinical study in cell and animal models, clinical trials are needed to determine whether these metabolic therapies can improve outcomes for children with high-grade glioma.

Could the glucose ketone index (GKI) become a simple way to monitor metabolic health across cancer and other chronic diseases?
This review explores growing evidence that mitochondrial dysfunction contributes to conditions including cancer, type 2 diabetes, cardiovascular disease, obesity, neurodegenerative disorders, and some psychiatric illnesses. The authors propose that the GKI โ calculated by dividing blood glucose by blood ฮฒ-hydroxybutyrate (BHB) โ may provide a practical way to monitor ketogenic metabolic therapy, which may help restore mitochondrial function.

Five-year nutrition studies are rare, but long-term results matter.
A recent study examined whether a digitally delivered, carbohydrate-reduced nutrition program designed to promote nutritional ketosis could help adults with prediabetes maintain long-term metabolic health.
This 5-year extension study followed 96 adults with prediabetes enrolled in a continuous remote care program combining individualized carbohydrate-reduced nutrition therapy with digital health coaching and medical support.
๐น 22% achieved normoglycemia after 5 years, while only 13% progressed to type 2 diabetes.
๐น Participants maintained an average 5.8 kg (5.3%) weight loss, with 47% achieving at least 5% weight loss.
