The IIT story people know is software, campuses, and startups. The quieter one is this: relapsed blood-cancer patients in India no longer have to fly west for CAR-T / pay ~₹4 crore. IIT Bombay designed NexCAR19. Tata Memorial trialled it. CDSCO approved it.

Standard oncology relies heavily on imported drugs and protocols, a team at IIT Bombay (led by Prof. Rahul Purwar, alongside researchers Alka Dwivedi and Atharva Karulkar) spent over a decade developing India’s 1st indigenous CAR-T cell therapy: NexCAR19 (actalycabtagene autoleucel).

In partnership with Tata Memorial Centre (led clinically by Dr. Hasmukh Jain and Dr. Gaurav Narula) and spun off as ImmunoACT, the therapy moved from academic labs to clinical trials, ultimately securing CDSCO regulatory approval in Oct 2023.

What the Science Actually Does....

For patients with relapsed / refractory B-cell blood cancers (such as B-cell lymphomas and B-ALL, age 15+), doctors extract the patient's own T-cells via leukapheresis. The lab uses a viral vector to genetically engineer these cells to express a Chimeric Antigen Receptor (CAR) targeting the CD19 protein on cancerous B-cells, expand them, and infuse them back into the patient as a targeted living treatment.

Commercial US CD19 CAR-Ts (like Kymriah or Yescarta) used murine (mouse-derived) scFv binding domains, which carry immunogenicity concerns. The IIT Bombay team engineered a humanized CD19-directed construct.

In clinical studies, ImmunoACT reported a favorable tolerability profile with lower rates of severe high-grade Cytokine Release Syndrome (CRS) and little / no high-grade ICANS in the registrational studies, though mild-to-moderate CRS remains a common expected immune response.

Western list prices for CD19 CAR-T therapies run upwards of $400,000 (~₹3.8+ Crore) per patient. NexCAR19's product price was launched in the ₹30-40 Lakh range, ~1/10th the US list price.

NexCAR19 is not a universal cure, nor does it replace frontline chemotherapy / bone marrow transplants for all cancers. It is approved specifically for difficult, relapsed CD19-positive B-cell malignancies where prior standard lines of therapy have failed.

However, as India's 1st homegrown approved CAR-T therapy, it is a landmark translational achievement: taking advanced synthetic biology out of an academic lab, passing multi-center clinical trials at Tata Memorial, and offering Indian patients access to advanced cell-and-gene therapy at home at ~10% of Western list prices.

Bharat is rising. 🙏🙏
