There is a misconception that only amyloid-targeting treatments have been studied in Alzheimer's disease. The reality is that clinical trials have evaluated many, many treatments unrelated to amyloid over the past few decades. Please see the chart below from 2016, which shows the large number of non-amyloid-targeting treatments that were in trials ~9 years ago. With rare exceptions (acetylcholinesterase inhibitors, memantine), the non-amyloid-targeting treatments did not work.
Amyloid-targeting treatments are the only disease-modifying treatments that have demonstrated any clinical efficacy in slowing Alzheimer's disease so far in rigorous phase 3 clinical trials. Researchers/clinicians often talk about anti-amyloid treatments not because other types of treatments haven’t been considered or evaluated, but because some anti-amyloid treatments have demonstrated efficacy and are clinically available.
We do need even more effective treatments and researchers are continuing to evaluate many types of treatments in numerous trials. In 2024, there were 164 clinical trials assessing 127 drugs with a wide array of targets including neurotransmitter receptors, inflammation, and synaptic plasticity: (https://t.co/GpZMOC7fx6).
