The antiviral drug valaciclovir (Valtrex), given prophylactically, kept patients with SLE under treatment with anifrolumab (Saphnelo) from having herpes zoster attacks, researchers said, offering a potential alternative to zoster vaccination.

Some take away points:
- Overall 4% of patients flared
- Of those with a connective tissue disease (CTD) like lupus etc, half of those who flared needed medication adjustments
- 97% of CTD patients did NOT flare (VERY IMPORTANT number to keep in mind)
- CTD patients who were in remission were less likely to flare
- Patients on methotrexate were less likely to flare
- Stopping immunosuppressants to improve efficacy of the vaccine was more likely to be associated with flares (but most who stopped meds did not flare)
- The vast majority of flares were mild to moderate severity in CTD patients
- 0.5% of all CTD vaccinated patients had a severe flare (around one out of every 200 vaccinated CTD patients)
When making a decision on medical treatment (such as vaccines) it should be a decision on the risk of adverse events (like severe flare) vs benefit.
With the above data, I'll continue to highly recommend vaccination for my patients. I've seen too many deaths in my immunosuppressed patients from COVID (as recently as a few months ago).
However, in those who are not in remission, I'd discuss the possibility of not holding medications at the time of the vaccine (much less likely to flare if medications are not stopped), which would reduce the risk of flare, vs holding immunosuppressants to improve the effectiveness of the vaccine.
Bottom Line: Vaccinations are a personal choice. Do what you feel is best for your body.
