In Australia, Ambulance services are charged and not covered by Medicare. Where I live, $1,143 per km. People still call the Ambulance. Ambulance insurance is available and also covered by private medical insurance and motor vehicle insurance. There are various categories where service is free of charge - children, victims of violent crime, rescuers etc - and also discretion to waive fees for financial hardship. System works well - people who can afford to, pay for (or insure for) the service.
