So exciting to welcome back @MagnusHallandMD as a guest speaker to kick off our @MayoClinicGIHep fellows summer core curriculum!
The packed house with rapt attention is a clear testament to his teaching reputation & excellent delivery!
Our group was the first to define POSED (Post-Obesity Surgery Esophageal Dysfunction), a newly recognized motility disorder that can develop in a subset of patients after metabolic bariatric surgery.
In our latest @NGMJournal Neurogastroenterology & Motility study, we uncover a potential mechanism:
🔹 Low proximal gastric distensibility and elevated post-swallow intragastric pressure increase esophageal afterload
🔹 This leads to secondary peristalsis abnormalities, eventually progressing to primary motor dysfunction (resembling achalasia)
🔹 70% had abnormal FLIP findings despite normal or borderline HRM
📌 Clinical vigilance is key—patients with new-onset dysphagia after bariatric surgery should be evaluated for POSED, even when standard manometry appears normal.
Collaborative work with @KarthikRavi@OMGhanemMD@MayoClinicGIHep colleagues.

📌 New Insights into Dyssynergic Defecation by @MayoClinicGIH and Dr. Adil Bharucha
• 🔍 Integrated manometry + proctography = Seated proctomanometry
• 📊 Proctomanometry redefines our concepts of normal and dyssynergic defecation
• 🚻 Most DD patients have both impaired propulsion & relaxation
• 🧠 Consider biofeedback therapy guided by manometry

@DrViswanathRed1 Different system to what I use in my lab, but by color reference the pressure zone near the diaphragm and LES it at times same color as UES (typically 80-120 mmHg) - you rarely if ever see those pressures around and the crural diaphragm- suspect calibration or catheter issue
