A 33-year-old man with a known history of gout presented with a 1-year history of a growing right medial canthus lesion. Examination revealed a 1.5-mm subepidermal nodule overlying the medial canthal tendon (A), initially suspected to be an epidermal inclusion cyst. Excision and histopathologic analysis confirmed a gouty tophus, demonstrating feathery clefts from dissolved urate crystals and multinucleated histiocytes (B and C, hematoxylin and eosin staining, 10× and 40×, respectively). The patient had never been on urate-lowering therapy. His most recent uric acid level was 6.6 mg/dL (reference range, 3.4–7.0 mg/dL). Periocular gouty tophi, although uncommon, should remain in the differential diagnosis of subepidermal periocular nodules, particularly in patients with a known history of gout.

In our new EJAP paper with @MartinMacInnis, we found that the respiratory compensation point (RCP) agrees well with critical power (CP) if individual VO2 kinetics are accounted for (and poorly if ignored), but the CP model chosen also affects the agreement.
