The idea of specifying a piece of work in great detail up front, accurately predicting dev time, and then "failing" if you don't meet that prediction, is an extreme form of irrational thinking that simply does not, and never has, reflected the way software dev works.
Estimates are not predictions. They are a point in a probability field. Imagining that you'll hit that point all the time just means that you don't understand how probabilities work. A more accurate expectation is that you'll never hit it precisely.

It is a search for "better ways" based on 4 simple heuristics.
It's not a process or a role title or a flow chart.
It's not a matter of repeating an org structure or a calendar of events.
Your team member gets sick.
Your boss is on vacation.
Sr tech is in a meeting.
The SME/BA is out with customers.
You got a phone call from child care.
Someone stops by with a question.
These aren't special cases.
Is your work style resilient enough to work in normal life?
Agile is not a done deal. We haven't figured it out. There is no well-defined process that we can adopt. Remember, the first words of the Manifesto are "we are uncovering," NOT "we have uncovered."
The only way that I know to be successful at transitioning to Agile is to solve actual problems that the actual business has by applying Agile/Lean principles. The frameworks are an utter irrelevance, and drive organizations into faux Agile more often than not.
