When any cardiovascular risk score will provide confidence intervals for the population, we will make a small step forward from the current foggy version of risk assessment that it is offered. However, only having an individualized risk score can really change the prognosis of the individual patient and that type score does not exist unless driven by coronary imaging.
If you want to properly assess cardiovascular risk, the best option we have currently is Cardiac CT. From there you can start a preventative approach for the individual patient.
🧠🔬 Dual-Source Photon Counting CT for Spectral Evaluation of Coronary Atherosclerosis 💥🫀
The new frontier in cardiovascular imaging is already here — and it speaks the language of photons 📡 and spectral tissue characterization 🌈.
Photon Counting CT (PCCT) with a dual-source system brings together two groundbreaking technologies.
This combination enables advanced spectral assessment of coronary plaques in a single acquisition, with enhanced spatial resolution and native spectral sensitivity — no artificial post-processing needed.
🎯 Why is it so powerful for coronary atherosclerosis?
🔹 Native multi-energy acquisition
PCDs discriminate the energy of each individual photon 💎, allowing true voxel-based multi-energy imaging, without interpolation.
🔹 Tissue composition analysis of plaques
Spectral data allows differentiation between:
🟤 Lipid-rich plaque
⚪ Fibrous plaque
⚫ Calcified plaque
🔹 Early identification of vulnerable plaques
Low attenuation plaque (<30 HU), “napkin-ring” signs, and other high-risk features are better characterized thanks to detailed spectral information 📊.
🔹 Calcium artifact reduction
With virtual monoenergetic images (VMI), you can “navigate” through energy levels (e.g. 40–190 keV) ⚙️ to optimize contrast and reduce blooming artifacts 🧊.
🔹 Precise iodine quantification
Volumetric quantification of intra-plaque iodine, intraluminal enhancement, or perivascular uptake opens new paths for evaluating inflammation, neoangiogenesis, and capillary permeability 🔥.
🔹 UHR resolution
Thanks to sub-millimetric pixel sizes (0.1 mm) and no electronic cross-talk, even microcalcifications and thin fibrous caps can be detected with confidence 🧩.
🔹 One scan, multiple insights
A single coronary scan yields:
Standard morphological images
Spectral maps (iodine, VMI, Z-effective, electron density)
Advanced reconstructions (plaque maps, subtraction images, etc.)
🚀 PCCT + Dual Source = Coronary CT 3.0
A technology that enhances diagnosis, anticipates clinical events, and makes the invisible visible.
Key benefits in:
✅ Prevention
✅ Risk stratification
✅ Monitoring anti-atherosclerotic therapies
✅ Clinical and translational research

💉Targeting Triglycerides: The Future is Here
👉From ApoCIII to ANGPTL3 and FGF-21 analogues, new siRNA and ASO therapies are transforming the landscape of hypertriglyceridemia management.
📉 TG reductions reaching up to 80%!
👉Mild to severe HTG, monogenic or multifactorial — new options are emerging across the board.
1️⃣ Mild to moderate HTG (first slide):
👉siRNA vs ANGPTL3 vs ApoCIII:
In mild-to-moderate HTG, agents like Plozasiran and Zodasiran are showing up to 63% TG reduction in phase 2b trials.
👉Weekly vs quarterly dosing, different targets, different strategies.
Stay tuned as these agents move toward phase 3.
2️⃣ Severe HTG (FCS/MCS - second slide):
👉Managing sHTG and FCS is no longer science fiction.
Volanesorsen (ASO) leads with up to 88% TG lowering, while siRNA agents like 👉Plozasiran show promise with 80% reduction in FCS.
New frontiers, real hope for patients at highest pancreatitis risk.
