How do you dose therapeutic enoxaparin in obesity?
Patient has a BMI of 60 kg/m^2 (166 kg) and is being started on a warfarin bridge with enoxaparin for an acute DVT.
I typically recommend dose-adjusting to 0.8 mg/kg Q12H with BMI > 40, with Anti-Xa monitoring.
