With a rising awareness of the oral microbiome, I have lots of patients that are experimenting with oral probiotics.
The good news is that it does seem to work. The bad news is that they regress back to their starting point (pathogenic state) a few weeks after they stop taking it.
Almost all of the literature points to the resilience of the oral microbiome, as well as the temporary changes from trying to shift it.
There are really only two schools of thought, and both work (albeit unpredictably so).
1) Sustained environmental pressure
Instead of throwing more bacterial strains at the problem, you need to think about the environment itself. Think: dietary nitrates, arginine, transient carb restriction, polyphenols, xylitol, etc.
2) Scorched earth + reseed
Literally blow it all up and then rebuild it. A sustained period of nuking the microbiome with sodium hypochlorite rinses, local antibiotic therapy, deep cleanings, and then reseeding it with live cultures, beneficial foods and supplements. This is an extremely advanced maneuver and I wouldn't recommend it for most people.
The unfortunate reality is that our diagnostic capabilities currently outpace our interventions by a factor of 10. Hoping that changes soon!
The biggest cheat code is to stop complaining and blaming. You’re giving your power away. Take ownership. Develop some agency. It may not be your fail but it is your responsibility. The best way to improve your life is to stop that shit and use that energy to improve your life.
