Reflections on SHA/SHIF: Bridging the Gap Between Theory and Reality
As I observe the rollout of the Social Health Authority (SHA) and Social Health Insurance Fund (SHIF), it’s clear there’s a troubling gap between the program’s ideals and the reality on the ground. SHA’s segmented healthcare model, inspired by systems in well-resourced countries, aims to improve healthcare efficiency. However, it assumes an infrastructure, staffing level, and digital connectivity that many Kenyan facilities lack. Our hospitals are often under-resourced and understaffed, and rural areas lack reliable referral networks, advanced equipment, and digital records.
For patients needing consistent care, such as dialysis or cancer treatment, SHA’s structure can complicate access, adding travel and financial burdens. Moreover, SHA doesn’t account for the unique health-seeking behaviors of Kenyans, particularly in rural or low-income areas, where access to healthcare is already challenging.
Another overlooked issue is workforce readiness. SHA assumes sufficient trained staff at each level of care, which isn’t our reality. Addressing Kenya’s specific needs will require phased adaptation, stronger infrastructure, and investment in workforce training.
To make SHA truly effective, we need a model that aligns with Kenya’s strengths, addresses our resource constraints, and better serves our diverse population.
