Most people think an outbreak ends when the last patient recovers.

It doesn’t.

An Ebola outbreak is only declared over after 42 days, which is two full incubation periods, with zero new cases. That number isn’t caution. It’s proof the last chain of transmission is truly broken.

Between the first suspicious fever and that final announcement sits one of the most disciplined processes I know in public health. Here’s how the 12 steps actually unfold 👇

𝟭. 𝗗𝗲𝘁𝗲𝗰𝘁. A nurse notices three unexplained bleeding cases in 48 hours and files an alert. Surveillance works because someone refuses to look away.

𝟮. 𝗖𝗼𝗻𝗳𝗶𝗿𝗺. The team checks the baseline. Three cases where there are normally none is far beyond expected. You declare an outbreak on numbers, not fear.

𝟯. 𝗗𝗲𝗳𝗶𝗻𝗲 𝗰𝗮𝘀𝗲𝘀. Before you count anything, every team agrees on what counts. No shared case definition means double counting and chaos.

𝟰. 𝗙𝗶𝗻𝗱 𝗰𝗮𝘀𝗲𝘀. Teams search clinics, pharmacies, burial sites and community networks. Every case you miss is a chain you never broke.

𝟱. 𝗩𝗲𝗿𝗶𝗳𝘆 𝗱𝗶𝗮𝗴𝗻𝗼𝘀𝗶𝘀. Early Ebola looks like malaria or typhoid. Only the lab turns suspicion into fact.

𝟲. 𝗗𝗲𝘀𝗰𝗿𝗶𝗯𝗲 𝘁𝗵𝗲 𝗼𝘂𝘁𝗯𝗿𝗲𝗮𝗸. Time, place, person. The epi curve turns scattered fear into a picture that tells you whether you’re winning.

𝟳. 𝗠𝗮𝗽 𝘁𝗿𝗮𝗻𝘀𝗺𝗶𝘀𝘀𝗶𝗼𝗻. A funeral? A caregiver? You follow the links until the source appears, because the source is the off switch.

𝟴. 𝗖𝗼𝗺𝗺𝘂𝗻𝗶𝗰𝗮𝘁𝗲. Silence spreads faster than the virus. Clear, jargon free messaging isn’t PR. In an outbreak, information is medicine.

𝟵. 𝗧𝗿𝗮𝗰𝗲 𝗰𝗼𝗻𝘁𝗮𝗰𝘁𝘀. Every contact is monitored for 21 days, Ebola’s maximum incubation. This quiet, exhausting work is what actually ends outbreaks.

𝟭𝟬. 𝗠���𝗻𝗮𝗴𝗲 𝗱𝗮𝘁𝗮. Hundreds of forms cleaned, linked and analysed every day. Messy data leads to wrong decisions. This is where strategy lives, not admin.

𝟭𝟭. 𝗜𝗺𝗽𝗹𝗲𝗺𝗲𝗻𝘁 𝗰𝗼𝗻𝘁𝗿𝗼𝗹. Isolation, PPE, safe and dignified burials, ring vaccination. Slowly the curve begins to bend.

𝟭𝟮. 𝗠𝗼𝗻𝗶𝘁𝗼𝗿 𝗮𝗻𝗱 𝗲𝘃𝗮𝗹𝘂𝗮𝘁𝗲. Are cases falling? Are contacts reached in time? You steer the response daily. You never set and forget.

From one nurse’s alert to a nation’s relief: 12 steps, hundreds of people, thousands of quiet decisions.

Outbreaks don’t end by luck. They end by system.

And that’s the quiet paradox of this work. It’s most powerful precisely when you never hear about it.
