@jpcostabel@iamritu Constrictive as septal bounce, high TDI velocity, respiratory variations, but what is the whole picture of the patient and how to approach as the CT will be reasonable to detect if we could give steroids or colchicine if there is less calcification or we go for pericardiectomy
Mid-ventricular HCM is one of the instances when Doppler can be misleading: despite high pressure gradient in the ventricle no high velocities are seen, which can be interpreted as non-obstructive physiology. Mid-systolic signal void is due to flow cessation explaining why gradients are not captured. The paradoxical low velocity flow then follows, which should prompt search for an apical akinetic chamber.

Yesterday @Tesslagra had an idea, and today we put it to the test!
Pt is a 78 year old with prior CABG and a known wide aortic root. The referring MD had difficulty engaging the LM.
This is our attempt using the hybrid JL/EBU guide! Worked like a charm.😀

✅ TEE is superior to TTE for PFO diagnosis
✅ Saline contrast injection from lower extremity (LE) works better than upper extremity (UE) for PFO
✅ Reason: Eustachian valve (EV), a vestigial structure at IVC-RA junction when prominent might direct UE injection away from fossa ovalis (FO) region
✅ This TEE example ⬇️ w/out a PFO shows nicely how this works. IVC flow (negative contrast) in last clip shunts contrast away from FO
