While the primary goal of diabetes medications is to regulate blood sugar levels, some have the added benefit of promoting weight loss—like #Mounjaro. Here's how fast this Rx takes to work. https://t.co/WwXBwVgvjZ
@BiggestComeback For the record my thread does not say to not exercise. Body composition is unquestionably important.
What I’m arguing against is the dogmatic repetition of “you’re not optimizing protein and exercise enough.”
Obese patients are always blamed for “not doing enough” even on GLP1
Exercise and extra protein will NOT make you lose dramatically more weight on GLP-1s like semaglutide or tirzepatide.
Influencers, fitness coaches, and even some doctors are overstating what the evidence actually shows—and it's time we talked about it. (1/10)
@BiggestComeback Totally is patient blaming to tell suboptimal GLP1 responders that the reason they didn’t lose enough weight is because they didn’t resistance train enough.
Focus on what actually matters:
• Staying on treatment
• Managing side effects
• Improving access
• Building sustainable habits
Exercise for health.
Eat protein for nutrition.
Don't confuse either with a magic lever for dramatically greater GLP-1 weight loss. (10/10)
Same old story. New packaging.
Obesity used to be:
"Eat less, move more."
Now it's:
"You're on the drug, but you're not optimizing hard enough."
Great results = your effort.
Poor results = your fault.
That's moral judgment, not medicine. (9/10)
@EricTopol@bterris@NYMag@intelligencer It’s a legitimate question. He’s already being treated with statin + Ezetimibe. Why further risk stratify him? Was he experiencing symptoms?
Here at ObesityWeek®
Favorite discovery so far. GLP-1 mouse study.
Food intake drops significantly in early treatment with GLP-1. Returns closer to baseline over time, along with increase in energy expenditure! Weight loss is maintained.

END/🧑🤝🧑 What can we do with this data?
➡️ Recognize obesity as a disease, not a personal failing
➡️ Expand access to effective evidence-based treatments — medications, surgeries, procedures — and move beyond political platitudes
➡️ Broaden public health priorities beyond heart disease to include sleep, liver, kidney, and joint health
This isn’t about shame. It’s about saving lives and improving healthspan.
