Data on GLP‑1 RA discontinuation in pregnancy are concerning: women who stop GLP‑1s before or early in pregnancy have higher gestational weight gain, more excess gain, and higher risks of preterm birth, gestational diabetes, and hypertensive disorders.
Preconception counseling must balance the benefits of GLP‑1 medication weight loss with the very significant weight gain rebound and cardiometabolic risks once these drugs are stopped for pregnancy.
