🔴MACROLIDES REVIEW.

EXAMPLES.

Macrolides are an antibacterial drug class whose members include:

👉azithromycin
👉clarithromycin
👉erythromycin
👉telithromycin
👉fidaxomicin

MECHANISM OF ACTION.

Protein synthesis inhibition.

👉Inhibition of bacterial protein synthesis by binding reversibly to the 50S subunit of the bacterial ribosome, thereby inhibiting translocation of peptidyl tRNA.

SPECTRUM OF ACTIVITY / INDICATIONS.

🔹streptococcal infections, syphilis, upper respiratory tract infections, lower respiratory tract infections, mycoplasmal infections, Lyme disease. Skin and soft tissue infections.

🔹telithromycin is effective against pneumonia.

🔹fidaxomicin is used against Clostridioides (formerly Clostridium) difficile infection.

🔹part of some H. pylori eradication protocols.

SIDE EFFECTS.

🔸nausea, vomiting, and diarrhea (especially at higher doses; fidaxomicin has a higher incidence of nausea)

🔸prolonged cardiac QT interval (especially erythromycin)

🔸ototoxicity (especially at higher doses)

🔸jaundice

🔸visual disturbance, liver toxicity (with telithromycin)

🔸note that erythromycin is also a motilin agonist, increasing the risk of GI side effects

DRUG INTERACTIONS.

Erythromycin and clarithromycin (but not azithromycin) inhibit CYP enzymes - increasing the risk of adverse effects for drugs metabolised by P450 enzymes. For example: there is an increased risk of bleeding with warfarin and myopathies with statins.

Macrolides should be prescribed with caution in patients taking other drugs that prolong the QT interval (SSRIs, antipsychotics, quinine, fluoroquinolones etc.).
