Be very careful what you wish for. That money isn’t suddenly going to go to private practices, it is going to be absorbed as a “savings”. Then, all these academic centers/non profits are suddenly going to be less interested in treating those difficult transfer patients and we are going to worsen our access problem. The system currently in place sucks, be careful we don’t trade it for a worse one
@benkohlmann Within a year. I wasn’t quite sure until I left for good and realized I just didn’t miss it. I really enjoy surgery and taking care of people, but not being treated like a tech while doing it. Took all the joy out of it.
@WilliamWallace What about higher genetic glucose set points like GKC-MODY? Slight FBS elevation as well as slightly elevated A1C, but no evidence of deleterious effects on organs and normal insulin sensitivity?
@RenoDrew The sad state of American medicine. Screening test for insurance approval for a FESS rather than exploring issue. Entirely likely that it is reflux/stress etc, but would be a great service to patient to have some one (the ENT) take a look and explain it.
“You get what you INSPECT, not what you EXPECT”. And right now we are inspecting bibliographies. Because we don’t have grade or board scores. As a result I have had med students tell me they work on their “research” and “volunteering” before they study. One point over passing is all they need to get in courses, and they know that
Some day we will have enough data on scans in asymptomatic people and they will then be of value. I have seen near fatal results from people who had surgery for something found on those scans that turned out to be normal variants. Our scans are currently “tested” on symptomatic people, not asymptomatic so we don’t know what to do “abnormal” findings.
Physicians are powerless because they are busy taking care of patients for 60 hours a week. The professional organizations that have claimed to be “working for them” have been absolutely negligent in doing so, all while convincing the rank and file that their needs are being cared for on Capitol Hill.
Not really Chris. Biochemistry doesn’t really have much to do with “common sense”. The problem is, if we are being honest, we don’t KNOW what makes our bodies work at a mechanistic level with any confidence. Should we just wait around not treating people until we can really “understand”?

@TakeWeightOffMD Good advice….except. Attorneys get paid by the hour. They also have the documents with the potential problem sitting in front of them at their leisure. I have my 15 minutes to evaluate my patient and come up with a plan. No time to have a discussion with an AI🙄
@DrDeepMD No VIP phone line, one phone number gets you an appointment with the correct physician whether you are the president of Microsoft or the guy who sweeps floors at Costco. ALL my patients are VIPs!
@rbarbosa91 Yup. Same with “video visits”. I had a post op brain tumor patient come in with one leg 3x size that he didn’t notice until I pointed it out. This is the value of an actual physician.
As an American Airlines Executive Platinum, I applied for @Delta’s Status Match Challenge ~2 weeks ago to shift my travel loyalty. Frontline support says they have “no access” to it. Now my flight is this weekend, but approval takes up to 15 business days — and the 90-day MQD clock starts 24h AFTER approval. This setup makes planning impossible and feels like a bait-and-switch for status switchers. Tried DMing @Delta for escalation but “don’t have permission” error (your DMs appear restricted). Can the Medallion review team expedite or confirm? Seems crazy to try to get new Medallion members and then make it impossible to obtain? Anyone else run into this?
@RSE_VB Interesting study during Vietnam war. Navy carrier pilots had a higher heart rate during night carrier landings that they did while being shot at with AA fire up north. And they do carrier landings like that routinely in peacetime 🤔
