@fatwhitebloke My 6 penn'orth..It needs someone who both believes in and strictly enforces diversity values. My impression - CD was liked by her people, but not sufficiently respected to follow her lead on issues of discrimination. It needs the equivalent of a Sir Alex Ferguson.
So, it would appear that the inventors missed it, the manufacturers missed it, the medics missed it, the researchers finally identified it. The organisations of most if not all will have expressed a commitment to diversity. What went wrong then, and where else is it going wrong?
