Infants used to have a higher risk of dying than 80-year-olds—

Infancy used to be an extremely dangerous time of life.

As the chart shows, around 20% of girls in Sweden died before their first birthday in the 18th century. This was higher than the risk among 80-year-olds — at that age, 10% to 20% died each year.

Since then, the risk of dying has reduced across all ages, but the reduction has been most profound for infants. Rates have fallen 100-fold.

This progress has come from improvements in hygiene, clean water and sanitation, vaccination, nutrition, neonatal healthcare, and surgery.

The data comes from the Human Mortality Database and the United Nations World Population Prospects. I’ve shown data from Sweden, which has the longest historical records, stretching back to the 1750s.

This preprint was published by the Nordic research group led by @anders_hviid assessing the benefit of monovalent XBB.1.5 Covid-19 mRNA vaccine across Denmark, Finland and Sweden with respect to covid-19 related hospitalizations or covid-19 related deaths.

They used a rigorous, tight, trial emulation design that matched each boosted individual precisely with another individual by sex, age (5yr group), # of previous vaccine doses, # of comorbidities, and priority vaccination group.

This matching strongly adjusted for any potential confounding to these key factors, and ensured that any differences were not due to differences in these factors between those getting boosted or not.

Each boosted and matched non-boosted was then followed up based on a common index date (thus also controlled for potential calendar date confounding) for up to 12 weeks, comparing the groups in terms of covid-19 hospitalizations and deaths.

They computed both relative risk reduction and absolute risk reduction.

Overall they found 60.6% relative reduction of covid-19 hospitalization (-191.2 per 100k absolute reduction) and 77.9% relative risk reduction for covid-19 related death (-109.2 per 100k absolute reduction)

They split results by country/sex/ age/previous dose/variant/whether given concurrently with flu or not, and results were consistent in these various subgroups, with similar relative risk reduction but of course higher absolute risk reduction for the groups with higher baseline risk.

They did a sensitivity analysis leaving out the first 3 weeks after vaccination to adjust for temporal healthy vaccinee effect, which found similar results, so they were not driven by the temporal HVE.

They also summarized results by 3 week groups to measure the waning effect over time, of which there was some but not very much at all (see below).

Remarkably well done study of booster effectiveness based on cutting edge trial emulation design, and with enormous sample sizes enabling high statistical power and the ability to precisely match boosted and unboosted with respect to many key potential confounding factors, which all adds to the credibility of the results.
