Improvement toolkits & good practice databases are useful but they only take us so far. As much as 80% of the knowledge in an organisation isn't written down. It's not a failure of documentation. it's to do with the nature of the knowledge and the way it gets spread. In our change & improvement efforts, we typically focus on what we want to start to do, or do more of. We find it harder to stop doing things.
