1/8) How I passed European Specialty Examination in Nephrology "ESENeph"?
In this thread I attach my experience (I attached it as photos because twitter didn't accept to view some the tweets)👇🏼

🐁 The tiny 10 gm Pygmy Mouse achieves an incredible VO2max >250 ml/kg/min.
This is a great demonstration of allometric scaling laws at work: smaller animals have much denser mitochondria enabling greater VO2; this is also why they have higher BMR/mass

🩸 Paraprotein Evaluation 🩸
SPEP, SIFE, SFLC, UPEP, UIFE.
What do these abbreviations mean & how does one arrive to the land of paraproteinemias?
Here is a tweetorial for the internist diagnostician, in collaboration with @cullen_lilley!
@DavidJuurlink I use Valsartan.
Because it also lowers CRP.

Ironically, Losartan is one of the only ARBs that doesn't lower CRP.

@IM_Crit_@drshahrul80 The big question is why do you need to do this, so many other talented people can do the job, why should you be shortening your own lifespan for it
@ezm4n @DrDzul@Khairykj@DrZalihaMustafa Haha, the question of how to and why treat the uncivilised, I guess in this society you need to choose who you serve
@sanusizulkifli @victor_lyh Actually pay on nhs is not good as well as far as I know, but at least you work in a developed health system and it is a stepping stone to better pastures afterwards

IGNORE THE MCH!
I don't know about you, but our institution provides the mean corpuscular hemoglobin (MCH) with the CBC.
Not only is it a virtually useless (and redundant) parameter, but it often gets confused with the much more meaningful MCHC.
Sadhguru started having a headache since 1st week of March.
He neglected it.
He even attended and performed at Maha Shiva Rathri night on March 8.
His headaches got worse.
He neglected it.
A week later headaches became excruciating. On 15th March he checked himself into a hospital.
The neurologist identified a large few weeks old bleed in the brain and areas of new bleed.
Emergency admission and management advised.
Sadhguru neglected it because he had India Conclave Meeting to attend.
Two days later, after he started losing consciousness and losing control of one limb, he checked in again with the Neurologist.
Right after admission his consciousness worsened and new CT scan showed brain swelling and compression of brain against the skull.
Sadhguru then said "Now is the time to operate."
Emergency medical science and skilled surgical intervention saved him.
I hope he recovers well, and wish him the best.
Remember we are all human.
It was very foolish of Sadhguru and especially his advisors to neglect the doctor's advise and convert this easily controllable situation into a life threatening one.
I learned it the hard way when I had a near fatal accident.
Don't make your work your life.
Consciousness means "bloody hell listen to your doctor." Nothing else.

You are rotating on the Procedure Service and your team is asked to perform a routine “therapeutic” paracentesis on a patient with cancer. You walk into the room to meet the patient and this is what you see.
This finding should generate a hypothesis.
