@dan26wales Old people = wear and tear across multiple systems but can also suffer catastrophic component failure at any time as a natural part of the vehicles disintegrating trajectory.
@dan26wales Prof Robin Taylor has some really excellent talks on this, the phenomenon being known as discontinuity of care, focusing on a single issue, ignoring the over all trajectory of the patients illness e.g. pneumonia in end stage dementia.
@liambarker I'm northern as fuck and proud of it, never had it used as an excuse like that.Appalling. But its that attitude that when taking referrals I no longer ask for diagnosis etc, I ask how I can help. Get loads more info, context and actual clinical concern rather than just a referral
