I talk about 'rare' all the time but this is NEXT LEVEL rare...
Throrotrast (colloidal suspension of radioactive thorium-232 dioxide) has been discontinued since the early 1950s!...
Discontinuation was driven by recognition that Thorotrast, once injected, was retained permanently in the reticuloendothelial system (liver, spleen, bone marrow, lymph nodes)... with a biological half-life of several hundred years, resulting in lifelong chronic alpha-particle irradiation...
German Thorotrast Study: median life expectancy was shortened by 14 years, liver cancer standardized mortality ratio exceeding 12,000–16,000, excess cancer risk persisting for over 50 years after injection... scary stuff!
Super duper rare but a classic!
Erdheim-Chester Disease!
Non-Langerhans histiocytosis with a constellation of findings you won’t forget once you’ve seen them:
🦴 Symmetric sclerosis of long bone diaphyses/metadiaphyses (sparing epiphyses)
🫘 Bilateral renal soft tissue infiltration → “hairy kidneys” on CT
🫀 Soft tissue encasement of the aorta → “coated aorta” sign
🧠 CNS involvement — pituitary stalk thickening, cerebellar/brainstem lesions
🔶 Orbital infiltration causing proptosis
⚡ Key associations:
→ BRAF V600E mutation in ~50%
→ Diabetes insipidus is a common presenting symptom
→ 18F-FDG PET/CT is invaluable for staging & treatment response
📸 When you see bilateral perirenal soft tissue + long bone sclerosis on a CT — think ECD!
Woooaaaahh that’s an often mentioned but rarely seen diagnosis in the spleen, pretty rare, often multiple, with malignant potential, needs splenectomy!
66 M with with essential thrombocytosis.
What is the diagnosis?
We’ll post the answer in 24h. Share companion cases with us using #CookyBites #263. We will RT the best cases!
