Intracranial aneurysms may occur at many locations, but overwhelming at the ACOM & PCOMM. Look hard at these regions!
Remember, you miss 100% of aneurysms you don't look for!
Remember these two regions by always saying to yourself before you sign off any angiogram report:
"Always Please Communicate Aneurysms!"
Always = Anterior
Please = Posterior
Communicate = Communicating
Hopefully this will rupture any misconceptions about where to look for aneurysms!

Durante más de 20 años, una velocidad pico sistólica (PSV) de 125 cm/s en la arteria carótida interna era el punto de corte para diagnosticar una obstrucción ≥50%.
✅ Pero en 2021, un estudio de validación del IAC (Gornik et al., Vasc Med) demostró que ese umbral sobrediagnosticaba estenosis significativas.
📈 Nuevo umbral oficial (IAC 2023, JASE 2026): PSV ≥180 cm/s

IVIg for CIDP: Start with the loading dose (2 g/kg), then give 1 g/kg every 3 weeks. Schedule a follow-up visit in 9-12 weeks. Treatment response should be assessed using the INCAT score (at least 1 point improvement) and/or the MRC sum score (>2 points). If there is no response, reconsider the diagnosis, as it is unlikely to be CIDP. Do not use symptom improvement or “halted progression” as proof of response. Refer to a @gbscidp center of excellence if there is no response or if there are doubts about the diagnosis.
