Each care setting uses a different EHR which means when patients transition the EHRs don’t talk to each other. Multiple efforts are underway I believe to remedy this, but not fast enough!

During rounds yesterday, non-responsive end-of-life nonagenarian in our #LTC home on SC hydromorphone ATC started waving her shoulders and arms up in the air after the recreational therapist played Cuban music for her as her son instructed.
We all watched in awe!

During #LTC rounds today I noticed RN has word vocabulary in 5 different languages (Russian, Spanish, Portugese, Cantonese, Indian) other than her own as this is important for her residents with #dementia!
I wonder how much of what we “medicate” is communication barriers.
The cost of not keep up with vaccine boosters is too high individually and systemically. Pandemic fatigue & lack of public awareness of the science behind boosters is making many hesitant to get their booster shot.
