At Segue, we believe it's just as important to focus on the mental and spiritual aspects of a patient's recovery as the physical. Studies have shown that a holistic approach can help with pain management, & religion plays a prominent role for many.

Last month, we had the opportunity to come alongside a patient discharged from Anderson Hospital in Meridian, MS who needed prescription & utility assistance + personal care items. Together, hospital staff, our NP and Ministry Team were able to meet all this patientโs needs.
82% of hospitals throughout the country received some penalty from Medicare because of their readmission rates. With Segue's help, we can keep on caring and change that. https://t.co/2WSD8vN8jB via @RCJournal
Sometimes, it's hard to see the light in a situation, whether it's a long-term illness, marital problems, etc. We've compiled a list of Scripture-based books to encourage you when that happens. Because no matter what we are going through, we are not alone. https://t.co/xjqI9FFh0x
Last month Segue TX proved the extent of what it means to love our neighbors as ourselves. When a patient couldn't find transportation for his appointment, our care coordinator volunteered to drive him, stopping for groceries too. Thank you for responding with the love of Christ!
When ministering to patients, the circumstances can change rapidly, requiring us to adapt to the need of the moment. This past month we saw that first-hand with a patient and their family in Memphis. Our Ministry team and NP Cherish responded quickly, offering support and help.
This summer, we extended our care to Texas, starting with Texas Health Presbyterian Hospital Denton. We feel truly blessed to be able to serve the community of Denton, and we look forward to doing so in the months ahead.
Do you have questions about our transitional care program? You're not alone. We prepared a list of frequently asked questions that are available to you on the blog.

While our medical team serves our patients, the ministry seeks to serve Segue. One way they do so is through staff training, which prepares us to care in a Christ-like manner. A focus recently was discernment & listening: an act of love that goes with the attitude of Christ.
Earlier this year, we added two new hospitals to our network in Mississippi: @GLHnetwork and Baptist Golden Triangle. We have already seen God at work in these communities over the past several months, and we are eager to serve the residents of Greenwood and Columbus.
In times of difficulty, it can be hard to see and remember God's plan for us. However, we can find hope in knowing that His ways are higher than our own.
If ever you need to speak with someone this week, please know that our team is always available.
As our Care Coordinator was contacting patients, she recognized one with a need for prayer and encouragement in the face of a dire prognosis. She took her time to speak & share with this patient, & she passed the information to our Ministry Team. This is how we keep on caring.
Each month we submit a ministry report to our partners to show exactly how we kept on caring for our patients across the network. In July we focused on this verse from 1 John. Will you please join us in praying for our care team as we head into the new week and finish out August?
Our new website is now live! We encourage you to visit it to see where we are located, the services we provide and some frequently asked questions. Head there now to see how we keep on caring.

Empathy and human connection matter more than ever in a technology-focused healthcare industry, and we couldn't agree more. https://t.co/3yea1BZyoh via @HC_Finance
If you've ever wondered about the difference between home health and transitional care, you're not alone. Our Care Coordinator Claire Shamblin writes about the differences between the two--and how both are important--on our site.

We're halfway through the year, and what a year it has been. We have seen how God has provided, and we are humbled and honored to serve our patients across our network through both our medical and ministry teams as Christ has called us to do.
Last weekend we celebrated our nation's Independence Day. Yet as Christians, our citizenship is not here on earth but in heaven. Will you pray for our nurse practitioners and care coordinators, that they live out that truth while serving our patients as we have been called to do?
This month we had the opportunity to meet a need before a patient was even officially a Segue patient. He had undergone a serious medical procedure and needed a wheelchair. Since the patient had no means to purchase one, it was an opportunity for Segue to respond.
We recently had the opportunity to help a younger patient after discharge. He needed a walker and crutches, but insurance only covered one. Seeing that he would need the other, our NP turned to our Ministry Team. Within 24 hours, the patient received the crutches we had ordered.
