@MadanNarayanan Well that’s ridiculous. If you have a properly managed and working epidural you should NOT need to have pt’s on an opioid PCA. Agree with @amit_pawa. A solid NO here.

We made it a habit to always pre scan. I keep it simple with marks. First, parasag to count, mark ~2 interlaminar spaces preferring levels w/good ant. comp. for a more “open” space. Then, transverse & mark midline if can’t palpate.

@DBelardoMD Thank you from all anesthesia out there. Re the other comments on here. I don’t think this is a specialty problem. I think it’s more of a generational/institutional norm. We’ve sent pt’s to their PCPs/cards and got back their own one page with a ✔️ by “cleared” and no other info.
