🔍 Not all pouch symptoms after IPAA in UC = pouchitis
Patients with urgency, frequency, abdominal pain, or difficulty evacuating may have:
🟢 Inflammatory
🔵 Structural
🟣 Functional
📌 If fecal calprotectin is low, the patient is in clinical remission, and pouchoscopy shows significant mucosal healing (>50% endoscopic improvement), symptoms may not be driven by active inflammation.
📌 Structural causes should be considered, particularly during the first year after pouch surgery. Evaluation may include: 🩻 MRE/MR pelvis 📷 Pouchogram 🔬 Pouchoscopy 📈 manometry 🌀 MR defecography
📌 When inflammation and structural abnormalities have been excluded and manometry is unremarkable, a functional pouch disorder (irritable pouch syndrome) should be considered.
💡 The key message: Not every symptomatic pouch needs escalation of IBD therapy. Identifying the underlying mechanism is essential for targeted treatment.
