Design principles act as guidelines for writing better software:

🔹 SOLID: Helps create systems that are easy to maintain, extend, and understand.
🔹 DRY (Don't Repeat Yourself): Avoid code duplication to ensure maintainability.
🔹 YAGNI (You Aren’t Gonna Need It): Focus on what's needed now, avoiding premature optimizations or over-engineering.
🔹 KISS (Keep It Simple Stupid): Make things as simple as possible so they’re easy to use and fix.

🔸 Testing: Unit, integration, and automated testing are critical to ensuring software reliability.
🔸 CI/CD ensures that code is frequently integrated, tested, and deployed, reducing bugs and improving delivery times.

Understanding how to use version control systems like Git is a non-negotiable skill. Working with branches, pull requests, and collaboration workflows, such as GitFlow, is essential in almost every development environment.

Efficient coding often depends on selecting the right data structures and algorithms. Understanding basic concepts such as sorting, searching, graph traversal, and hash maps will provide a strong foundation for problem-solving. Also, Big O Notation.

𝟲. 𝗗𝗮𝘁𝗮𝗯𝗮𝘀𝗲𝘀 𝗮𝗻𝗱 𝗦𝗤𝗟

Whether relational (SQL) or NoSQL databases, knowing how to design efficient database schemas and optimize queries is essential. A deep understanding of databases, including indexing and transaction processing, can significantly enhance performance.

𝟳. 𝗦𝗼𝗳𝘁𝘄𝗮𝗿𝗲 𝗔𝗿𝗰𝗵𝗶𝘁𝗲𝗰𝘁𝘂𝗿𝗲

A sound software engineer should be familiar with common architectural patterns. Whether building a simple monolithic application or designing distributed systems with microservices and event-driven patterns, understanding trade-offs is critical.

𝟴. 𝗦𝗲𝗰𝘂𝗿𝗶𝘁𝘆 𝗕𝗲𝘀𝘁 𝗣𝗿𝗮𝗰𝘁𝗶𝗰𝗲𝘀

Security must be part of your development process from day one. Familiarity with common vulnerabilities, secure coding practices, encryption, and authentication methods is crucial for protecting both the software and its users.

Confused about the difference between ‘AI’ and 'agentic AI'?

Let’s break it down in the simplest way possible.

𝗥𝗲𝗴𝘂𝗹𝗮𝗿 𝗔𝗜 (like early chatbots):
• Gives you a one-time response to your query
• Works with what it already knows from training
• Can't access external tools or information
• Forgets your conversation as soon as it's over
• One question = one answer

Think of regular AI like asking a smart friend a question. They'll give you their best answer based on what they already know, but that's it.

𝗔𝗴𝗲𝗻𝘁𝗶𝗰 𝗔𝗜 (like modern assistants):
• Has a specific role and goal
• Can break down complex problems into smaller steps • Uses tools to get information or perform actions
• Remembers previous interactions to improve over time
• Can reason, plan, and adjust its approach

An AI agent is more like hiring a personal assistant who can not only answer questions but actually complete tasks for you by making plans, using different tools, and learning from experience.

The magic happens through a process called "𝗥𝗲𝗔𝗰𝘁" (reason + act):
• The agent thinks about your request
• Plans how to solve it
• Uses tools like search, databases, or APIs
• Checks if the result makes sense
• Adjusts if needed until the task is complete

I am angry today. I hv a rare off day at my hospital. An unknown gentleman had been frantically msging me in Twitter throughout the last week, with panic.
His son has been admitted to a top private Chennai Hospital, owned & run by a General Surgeon. The son (call him X) had been admitted under this surgeon at the posh hospital. X had been advised the removal of an important urological organ, under him by the surgeon. Cd I come & see X?
I did visit X at the corporate hospital this morning. I was flummoxed. Not only was this a rare Urological disease, but I am certain the General Surgeon does not even know the name of the disease.
After seeing the scans,& more importantly the BMW which X had parked at the parking lot( CCTV), the surgeon had advised surgical removal of a vital uro organ. Of course, the bill will come later.
The nurses & the duty dr ( surprisingly) recognised me by my Twitter DP,& I showed them my ID. I was permitted to talk to/examine the pt & go through the scans.
It is a rare urological disease curable by drugs & no surgery- least of all, organ removal is necessary. I dragged the father aside, explained the disease, & requested him to get a discharge & get admitted to a hospital where a Urologist is available.

There is a huge outbreak of infectious hepatitis from a single village in Kerala. Multiple hospitals are dealing with many patients, young and old, coming in with jaundice and hepatitis due to Hepatitis A virus infection.

My Unit is also treating many uncomplicated cases, but also patients who are severely affected with acute liver failure due to Hepatitis A virus.

Strangely, many complicated hepatitis A patients are presenting with kidney failure along with liver failure.

Acute liver failure is loss of liver function that occurs quickly — in days or weeks — usually in a person who has no preexisting liver disease. Acute liver failure can cause serious complications, including bleeding and increased pressure in the brain. It's a medical emergency that requires hospitalization. Depending on the cause, acute liver failure can sometimes be reversed with treatment. In many situations, though, a liver transplant may be the only cure.

Hepatitis A outbreaks occur due to contaminated water sources, unsafe handling of food or poor hand hygiene of food handlers - watch out for those street vendors and water sources.

According to the World Health Organisation’s Global Hepatitis Report 2024, India has among the highest burdens of viral hepatitis — tied with tuberculosis (TB) as the second largest infectious killer in the world. Covid-19 was the number one infectious killer.

There are five hepatitis viruses that cause liver disease in humans. Hepatitis A and E are transmitted through contaminated food and water, and result in few weeks of self-limiting symptoms such as fever, abdominal pain, jaundice, dark urine, and sometimes itching and pale stools.

Hepatitis B (HBV) and hepatitis C (HCV) spread through blood and bodily fluids. Some of these viruses may result in a short-term infections which resolve on their own. But others can lead to chronic life-long disease, cause cirrhosis (permanent scarring of the liver) and increase the risk of liver cancer.

HBV can be prevented by vaccination, while HCV can be cured with medication. Hepatitis D (HDV) can only infect those who are already infected with HBV. It can thus be prevented by vaccinating against HBV.

India has the world’s second highest prevalence of viral hepatitis cases, accounting for over 11% of the global burden.

Remember these:
✅Hepatitis A is vaccine preventable.
Minimum age is 12 months.
Live vaccine is single dose.
Inactivated vaccine is two doses.
Adults and seniors must also take it if living in endemic areas (like Kerala).
Cost is 1800 - 2500 INR for each dose.

✅Hepatitis B is vaccine preventable.
It is a three dose vaccine.
Check your HBV status
HBsAg - if positive, it means you are infected. Please see a doctor for further evaluation and need for treatment (all HBsAg +ve patients do not require treatment)
If HBsAg is negative and anti-HBs (antibody to HBV) is negative, then take three doses of vaccine
If HBsAg is negative and anti-HBs is positive (but titer is less than 100) then take single booster
If HBsAg is negative and anti-HBs is positive and titer is >100, then you are well protected

✅HCV has no vaccine, but it is curable with simple and effective, safe medications given for 3 to 6 months. Check anti-HCV status. If it is negative, then you do not need any further evaluation. If anti-HCV is +ve, then please see a doctor who will then assess further. The doctor will check if virus is active in the body by testing HCV-RNA in blood. If RNA is detectable, then they will go on to check the type of HCV (there are many sub-types) and provide targeted antiviral treatment for cure and also look at further follow up based on presence or absence of underlying chronic liver disease (please note: anti-HCV test may never get negative even after viral clearance with drugs, but do not worry).

Please note: Jaundice is NOT a disease. It is THE SYMPTOM of a disease. Jaundice can be due to liver or non-liver disorders.
