For primary prevention, moderate-dose statin therapy is recommended for those aged ≥40 years with diabetes, although high-intensity therapy should be considered in the context of additional Atherosclerotic Cardiovascular Disease (ASCVD) risk factors.
Current guidelines recommend that in people with diabetes who are at higher cardiovascular risk, especially those with one or more ASCVD risk factors, high-intensity statin therapy should be prescribed to reduce LDL cholesterol by ≥50% from baseline and to target an LDL cholesterol of <70 mg/dL (<1.8 mmol/L).
It may also be reasonable to add ezetimibe or proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor therapy to maximum tolerated statin therapy if needed to reduce LDL cholesterol levels by ≥50% and to achieve the recommended LDL cholesterol target of <70 mg/dL (<1.8 mmol/L).
For secondary prevention, high-intensity statin therapy is recommended for all people with diabetes and ASCVD to target an LDL cholesterol reduction of ≥50% from baseline and an LDL cholesterol goal of <55 mg/dL (<1.4 mmol/L).
Addition of ezetimibe or a PCSK9 inhibitor is recommended if this goal is not achieved on maximum tolerated statin therapy.
Bempoic acid, Inclisiran and Icosapent ethyl are newer drugs that can be used along with statins to meet LDL-C targets.
