Root cause: a migration script was constructing a sqlplus command and executing it via eval.
$ in the DB password got expanded by the shell → wrong password passed → login failed silently.
Removed $, migration worked instantly.

⚠️Beware of using !, $, or other special characters in passwords used by scripts.

Just spent >2 hours debugging a fresh installation where a DB migration failed silently.
Logs only said “could not create schema / tables” - no real error.
