@AnnalisaB My mum has recently been discharged with palliative care and I feel confident and comfortable that the support from the health professionals will again enable us to do what is needed. Have you read With the end in mind? Kathryn Mannix - I found it hugely reassuring x

@AnnalisaB We had my father in law home for end of life care in 2020. Support from hospital staff planning discharge and district nurses / carers when he was here was wonderful. It was a real privilege to care for him at the end and I’m really glad that we did.
