@David_Kopin @JessTuckerMD @tkoellin Agree. If there is FDG uptake in the septum, would attempt a biopsy. Also would consider a whole body PET to look for other areas of inflammation that may have a higher diagnostic yield for a biopsy. Having an ICD offers some degree of protection while waiting for biopsy.

ISTH guidelines recommend against the use of DOACs in patients >120kg or BMI>40. However there is some emerging evidence that DOACs may be effective even in obese patients. What is your practice? @YogenKanthi @GBarnesMD
@BPierce_MD, a former Wash U internal medicine resident and chief resident and current UPMC cardiology fellow, has tragically died without known explanation. He was exceptional in every way. His loss is devastating. I'm grateful for the opportunity to have worked with him.
