Diagnosis: Aspergilloma (mycetoma / fungal ball) in a pre-existing upper lobe cavity (most commonly from old TB).
Key Radiological Signs:
- Air crescent sign (Monod sign) — the crescent-shaped air collection around the mobile fungus ball
- Rounded opacity within a thick-walled cavity, usually in the upper lobe
Why it fits the history: Cough + weight loss from fungal colonization; patients can also develop hemoptysis.
Common Differentials:
- Cavitating lung carcinoma
- Rasmussen aneurysm (in TB cavity)
- Hydatid cyst (less common)
Management Pearls:
- Observation if asymptomatic
- Antifungals (e.g., itraconazole) if symptomatic
- Bronchial artery embolization or surgery for life-threatening hemoptysis
- Follow-up CT to monitor for complications
