We talked to our blood bank and arranged for the "youngest" units to be kept for our massive transfusion packs

Stored blood is acidotic (pH about 5), cold, hyperkalaemic, lacks platelets, lacks clotting factors, and results in hypocalcaemia. 2,3 DPG doesn't become active for at least 6 hours. Bank blood RBCs are stiffer and less deformable through capillaries. = unit of Lethal Triad!!

Red cells given to improve oxygen delivery but stored blood not designed for trauma & 2,3-DPG decline may render it useless. Blood products not designed for trauma.

Patient with suspected aortic rupture after Road Trauma.
Contained haematoma gave way while on CT scanner. Radiographer has added red colour to fresh contrast extravasation into mediastinum and pleural space. Poor outcome despite urgent surgery.

have been very impressed with dermal replacement using BTM biodegradable before skin graft. Seems quite robust from infection. Able to discharge early and manage as outpatient till SSG. No conflict of interest to declare.
