We agree that SHA does not necessarily need to own or directly operate every ambulance, and that the Dispatch Centre's role is to coordinate patients with available ambulance providers.
However, I think there is an important distinction between ambulance ownership/dispatch and the delivery of prehospital clinical care.
The fact that SHA contracts or empanels different ambulance providers does not make the clinical workforce irrelevant to the SHA emergency care system. The critical question is, who is responsible for the clinical care delivered to the patient before arrival at hospital, what scope of practice applies, what standards govern that care, and how is that clinical workforce integrated into the wider health system?

An ambulance is not simply a means of transport. It is a platform for delivering prehospital care.(It brings care closer to the patient since hospitals don't go to patients) Dispatch determines which resource reaches the patient, appropriately trained clinicians determine what happens to the patient once that resource arrives.

This is also why we think the discussion about paramedics should go beyond whether SHA itself will put every paramedic on its payroll. Kenya needs to determine how paramedics and other emergency care professionals are incorporated across the providers delivering the SHA emergency benefit, including appropriate clinical governance, professional standards, accountability, data collection and quality assurance.

The objective should therefore not simply be to build a system that gets patients to hospital. It should be to build an EMS system that provides the right clinical care, by the right professional, at the right time, and gets the patient to the right destination.

That distinction is important if we genuinely want emergency medical services to become an integral component of Universal Health Coverage rather than simply an ambulance transportation benefit.
