After both ECTRIMS2026 and American Neurological Association, I was reflecting on how #autoantibody-mediated neurological diseases are such precise clinical and biological entities. Few other diseases carry such homogeneity.
In 2023, @EoinFlanagan14@adrianbudhram1 and I proposed it would be better to split, rather than lump. We suggested that the antibody itself - especially when highly specific - should be the gateway to diagnosis.
The fundamental demographics, clinical features and biological characteristics differ so markedly (see table) based on the antigen
It’s a key theme across our two main 2026 reviews and calls for a rethink re nomenclature of these conditions.

The autoantigen defines so so so much! It must be put at the center.
Congratulations to our very own Ashley Denney (PGY4) who’s CAP Abstract on Microglial-Amplified Inhibitory Synaptopathy: Convergent Neuropathology in GlyR, GAD65, Kelch11, and NIF Autoimmunity was selected as the First Place Abstract at CAP 2026 🎉

A diferencia de la gripe y el virus repiratorio sincitial, el SARS-CoV-2 aún no ha consolidado un patrón estacional estricto ni predecible comportándose más como un virus social y continua manifestándose de forma ondulante.

Another high profile paper showing the formation of blebbisomes from another cell type and not even a citation of the blebbisome paper...... Really???? My talk at AAEV next month is going to be fun for me........... https://t.co/OuHrgGZKs3

Think COVID-19 is just a temporary respiratory bug?
A brand-new 2026 preprint study reveals how SARS-CoV-2 inflicts direct, lasting damage on the brain's microvascular system—even after mild infections. 🧵#FarUVC

#LongCOVID can cast a remarkably long shadow.
➡️ In this longitudinal cohort, 61% met criteria for post-COVID condition (PCC) at ~16 months, and 59% still did so at ~48 months.
Among those with PCC initially, an extraordinary 77% continued to meet criteria four years later. 1/
Could some #LongCOVID symptoms be driven by antibodies that turn against the nervous system?
➡️ Researchers found autoantibodies targeting proteins in the central and peripheral nervous systems in a subgroup of people with LongCOVID, particularly those with neurocognitive symptoms.
➡️ Remarkably, their IgG antibodies reacted with human brain regions and peripheral nerves, and some antibody levels correlated with neurological symptoms. 1/

Supportive of that view but……isn’t it amazing that only these #LGI1#antibodies produce this phenomenon? So so so specific. The question is why. That’s the real dilemma to solve
