Real world completion rates for chemoradiation in stage III NSCLC @ClinicalLung. 246/265 pts (93%) who started CRT completed it. Reasons pts did not complete: toxicity, other illness, progression. Durvalumab here associated with improved OS (HR 0.39).

Neoadj chemo/IO is a huge advance
But to think that the 3 cycles of neoadj chemo/IO is the perfect dose for each and every pt fails to recognize the nuancing we will need to achieve to keep moving the needle

We are already learning from all recent studies how to use:
Path CR vs not
TPS score
RVT to guide shared decision making with a lot more to come (MRD/ctDNA?) and we certainly need trials and trials

As the saying goes: the only constant thing in life is change- clearly true for thor onc!

But for at least some pts- IMO periop therapy might just have the most muscle!
