I resigned from Anthropic today. I spent the last three years doing pretraining research at both OpenAI and Anthropic. Neither company is acting responsibly. They are racing straight to self-improving superintelligence and gambling with our lives. More thoughts below.
🚨 10 striking conclusions from ESC 2026
1. HFmrEF is essentially gone.
ESC now defines HFrEF as LVEF <50% and HFpEF as ≥50%.
2. MRAs have broken the EF barrier.
Mineralocorticoid receptor antagonists now have a Class I recommendation in chronic HF irrespective of LVEF. Semaglutide/tirzepatide receive Class IIa recommendations for HFpEF with obesity.
3. SINGLE-AF: anticoagulation may start earlier than we thought.
In AF patients at intermediate stroke risk, DOAC therapy reduced adverse clinical outcomes by 69% versus no anticoagulation.
4. Severe TR is no longer just “give diuretics and watch.”
TRIC-I-HF: transcatheter tricuspid repair reduced HF hospitalisation and all-cause mortality compared with medical therapy alone.
5. Unexplained syncope deserves early prolonged ECG monitoring.
ASPIRED: immediate 14-day monitoring did not prevent recurrent syncope, but substantially improved arrhythmia diagnosis/treatment and was associated with lower mortality.
6. Intermediate–high-risk PE may be entering the catheter era.
PRAGUE-26: catheter-directed thrombolysis improved short-term clinical outcomes versus anticoagulation alone, without an obvious major bleeding penalty.
7. Cardiac arrest + no STEMI ≠ automatic immediate cath lab.
DISCO: immediate angiography after OHCA without ST elevation did not improve 30-day survival.
8. Being ≥70 is not a reason to abandon primary-prevention statins.
STAREE: atorvastatin reduced major cardiovascular events by about 30% in older adults without known CVD, although disability-free survival was unchanged.
9. AF ablation has a surprisingly large placebo component for symptoms.
PVI-SHAM-AF: ablation reduced AF recurrence, yet improvement in AF-related quality of life was not significantly greater than sham treatment.
10. Even the definition of MI has changed.
The 5th Universal Definition of MI replaces the traditional numbered framework with primary MI, secondary MI and procedure-related MI, while incorporating sex-specific troponin thresholds and greater use of imaging.

Treat HF across the EF spectrum, anticoagulate selected AF patients earlier, intervene on TR, monitor syncope sooner—and stop rushing every post-arrest patient without STEMI to the cath lab.

One mark of genuine spiritual maturity that does not get nearly enough attention is simple steadfastness.
We live in a culture that celebrates intensity, visibility, dramatic experiences, and quick results. Scripture places enormous value on something quieter: continuing faithfully when nothing feels exciting, when prayers seem unanswered, when obedience becomes costly, and when no one is applauding.
Paul says, “Therefore, my beloved brethren, be steadfast, immovable, always abounding in the work of the Lord” (1 Corinthians 15:58).
Steadfastness is not stubbornness. It is settled faithfulness rooted in the character and promises of God. It keeps praying when emotion is gone. It keeps opening Scripture when the heart feels dull. It keeps resisting sin when the battle has lasted for years. It keeps serving when recognition never comes.
James says, “Let endurance have its perfect result, so that you may be perfect and complete, lacking in nothing” (James 1:4).
That kind of endurance exposes shallow Christianity very quickly. Anyone can appear strong during a spiritual high. The deeper question is whether we remain faithful through ordinary Mondays, disappointments, unanswered questions, suffering, and long seasons where God seems to be doing very little.
Jesus said, “The one who endures to the end, he will be saved” (Matthew 24:13).
The Christian life is not proved by how loudly we begin. It is shown by whether grace keeps us clinging to Christ all the way to the end.
Spiritual health is not always dramatic. Very often it looks like a believer who simply refuses to let go of Christ.
SGLT2 inhibitors are the big one I don’t want sneaking into major surgery. Euglycemic DKA is the concern, and the glucose can fool you because it may not be dramatically elevated.
But honestly, I look at the whole diabetes med list.
SGLT2s, GLP-1s, metformin, sulfonylureas and insulin all have their own perioperative considerations.
The question isn’t just “does the patient have diabetes?”
It’s what are they taking, when was the last dose, renal function, how long are they fasting, and what operation are we about to put them through?

Sometimes, what we confidently call “spiritual” may simply be a misunderstanding of Scripture.

Christian, consider what the gospel means for your suffering. Every trial you endure in this life is temporary. None of it is the condemning wrath of God against you, because that wrath was borne by Christ in your place.
“Therefore there is now no condemnation for those who are in Christ Jesus” (Romans 8:1).
The believer may know grief, sickness, betrayal, persecution, poverty, loneliness, and death. God may discipline His children severely, and His providence may lead us through places we would never choose. But none of those things are hell. Hell is judicial wrath without mercy. For the Christian, that judgment has already fallen upon the Substitute.
“Having now been justified by His blood, we shall be saved from the wrath of God through Him” (Romans 5:9).
This means the darkest suffering you will ever experience is happening on this side of glory. There is no greater terror waiting behind death for those who are in Christ. There is no second payment for forgiven sin. There is no condemnation remaining after Christ declared, “It is finished.”
Paul could therefore look at enormous suffering and call it “momentary, light affliction” because he was comparing it with “an eternal weight of glory far beyond all comparison” (2 Corinthians 4:17).
Your present tears are numbered. Your pain has an appointed end. Your losses will not follow you into the resurrection.
“He will wipe away every tear from their eyes; and there will no longer be any death; there will no longer be any mourning, or crying, or pain” (Revelation 21:4).
For the believer, this broken world is the nearest thing to hell you will ever experience. Ahead of you is not wrath, but Christ. Not condemnation, but glory. Not an eternal night, but the presence of the God who purchased you with His own blood.
So suffer faithfully. The worst is temporary. The best has not yet begun.
