What Dr No is saying is that even when it is a doctor with first hand experience of the patient, the clinical process of diagnosing cause of death is not exact science, not by a long way.

Clinical diagnosis of cause of death (ie no PM) is often unreliable. Plus there is 'hot stuff bias' - tendency to favour a diagnosis when it's a 'hot topic' (see blog again).
