Quincke's Sign
1. Quincke's sign is the presence of capillary pulsations visible in the nail bed when gentle pressure is applied to the distal nail.
2. It is a classic sign of severe aortic regurgitation (AR) and occurs because of a wide pulse pressure caused by increased stroke volume and rapid diastolic runoff.
3. The sign appears as alternating blanching and flushing of the nail bed, synchronous with each heartbeat.
4. Quincke's sign is one of several peripheral signs of chronic aortic regurgitation, along with Corrigan pulse, De Musset sign, Duroziez sign, Traube sign, and Müller sign.
5. The underlying mechanism is high systolic pressure with low diastolic pressure, producing exaggerated pulsatile flow through peripheral capillaries.
6. Although highly suggestive, Quincke's sign is not specific and may occasionally be seen in other high–cardiac output states such as thyrotoxicosis or severe anemia.
7. In a patient with suspected aortic regurgitation, echocardiography is the investigation of choice to confirm the diagnosis, assess severity, and evaluate left ventricular size and function.
