ctDNA in colorectal cancer is no longer “one test, one time.” A recent @NatureRevCancer Clinical Oncology review summarizes how liquid biopsy is becoming a longitudinal tool, supporting diagnosis, treatment selection, response monitoring, resistance profiling, and postoperative risk stratification.
🧬 In metastatic CRC, plasma ctDNA can capture tumour heterogeneity better than a single-site biopsy and allows serial tracking of clonal evolution, useful for resistance monitoring and for anti-EGFR selection/rechallenge.
🩸 In earlier-stage disease, postoperative ctDNA is a strong prognostic marker and is being evaluated to guide adjuvant escalation or de-escalation strategies.
⚠️ Interpretation still needs rigor, ctDNA detection depends on tumour fraction and shedding (so “negative” isn’t always “no disease”), and clonal hematopoiesis (CHIP) can drive false positives.
📌 Take-home: not tissue vs plasma—smart integration with standardized workflows will be key for clinical adoption.
